It is just based on your outcome, although there is no such thing as good and bad luck. I always thought it was funny how people say "It was supposed to be sunny today..." It's not like it was supposed to, that's what was predicted. I still can't get my head around it. 1 shield in 30 rounds, where the expected outcome is a least 5, as I was lucky some of the rounds. I think an exception was made for the Sword of Decay, allowing Mages to wield it, because it was owned by Atticus Zeigfried. I tried to create a sword useable by another class for a similar reason (in-universe history), and Sandy told me that weapons needed to fit into the existing categories. (I think it was the Sword of Ancestral Hatred that I wanted useable by Barbarians and Rogues.) Quote

If it was usable only by Barbarians and Rogues, it wouldn't've been suitable for any Expert Classes since it doesn't fit into a normal weapon category. I find it easier that way, it'd get confusing without them. Quote
